Pottawattamie County releases 2026–2028 community health improvement plan

The plan was developed using findings from the 2024 Community Health Needs Assessment and extensive community input. It identifies two priority areas where collaborative action can have the greatest impact:

• Social Determinants of Health

• Mental Health

A collaborative approach to community health

Improving community health requires collaboration across many sectors. Pottawattamie County Public Health developed the CHIP in partnership with healthcare providers, community organizations, educators, residents, and regional partners to establish shared priorities and strategies for the next three years.

“The Community Health Improvement Plan represents the collective voice of our community,” said Maria Sieck, Administrator of Pottawattamie County Public Health. “No single organization can improve community health alone. This plan provides a shared direction that allows public agencies, healthcare providers, nonprofits, businesses, schools, and residents to work together toward common goals.”

The CHIP outlines measurable goals and evidence-based strategies aimed at reducing barriers to care, strengthening access to essential resources, and improving health outcomes across Pottawattamie County. It also builds on regional collaboration while addressing the unique needs of the county.

Looking ahead

Through 2028, community partners will work toward the goals outlined in the CHIP, using the plan as a framework for local initiatives, partnerships, and community investments. Future community health priorities will be informed by a reevaluation toward the end of the implementation period.
